Ann will assess the level of distress of each new patient and assist them and their family with any problem or difficulty they may face during their treatment.

Services provided:

• Assist with financial/insurance questions and concerns
• Transportation assistance
• Medication assistance
• Homecare and hospice assistance
• DME (durable medical equipment)
• Assisting patients in establishing primary care physicians
• Nursing home placement/Short term rehabilitation
• Distress management/psychosocial evaluations
• Advanced Directives/Living Will/Power of Attorney
