SOMC Rehab Services (maps and contact information) features a full continuum of rehabilitation services. Our CARF accredited inpatient programs, our outpatient programs and our home care services allow us to meet the rehabilitation needs of a patient at almost any level of recovery. The SOMC Rehab team prides itself on continuous communication between physician, nursing staff, rehabilitation staff and social services so a patient can make a seamless progression from one level of rehabilitation care to the next. The team management model allows the patient to optimize recovery.
