The goal of our program is to provide a continuation of care, which offers patients choices for enhancing the opportunities for completing their lives with dignity, purpose and peace.

Pharmacies
SOMC currently offers community pharmacy services at two locations in Portsmouth and one in West Union! Our Portsmouth pharmacies are located on the Main Campus and in SOMC’s Portsmouth Family Health Center, located on the South Campus.
Rehabilitation Center
SOMC’s Life Centers offer Outpatient Therapy programs including Physical Therapy, Occupational Therapy, and Industrial Rehab in a fully integrated space.
South Campus
SOMC’s South Campus is home to the SOMC LIFE Center, SOMC Friends Center and the Medical Office Plaza.
Family Health Centers
SOMC offers family health services in three convenient locations: Portsmouth, Wheelersburg and Waverly.
