Hospice Services

SOMC Hospice develops a coordinated, individualized program of care for each patient and family by providing physical, emotional, social and spiritual support.

Nursing

Hospice nurses provide compassionate, personalized care focused on comfort and quality of life. They help manage symptoms, coordinate care, educate families, and provide support throughout the hospice journey. Through SOMC Hospice, you will be assigned a nurse case manager that will make routine visits on a weekly basis, or more often as needed. They will work with your physician to manage symptoms you may be experiencing.

Pain and symptom management

Hospice focuses on managing pain and other symptoms to promote comfort and quality of life. Our care team works closely with patients and families to develop a personalized plan that addresses physical discomfort and provides peace of mind.

Home health aide

Hospice home health aides provide compassionate, hands-on support with personal care. They help patients maintain comfort, dignity, and companionship while providing families with additional support at home.

Spiritual counselor

Our spiritual counselors offer compassionate support to patients and families as they navigate the emotional and spiritual aspects of the hospice journey. They provide a listening presence, comfort, and guidance based on each person’s individual beliefs and wishes. The spiritual counselors can provide this spiritual support in coordination with the patient’s own religious network.

Medical social services

Hospice social workers provide emotional support, guidance, and resources to patients and families throughout the hospice journey. They can assist with coping, care planning, family concerns, and connecting families with helpful community resources.

Counseling

When a family member is dying, everyone needs support. SOMC Hospice offers supportive counseling and “an opportunity to talk it out” for patients and families.

Volunteer services

Hospice volunteers provide companionship, comfort, and practical support to patients and families. They offer a caring presence through activities such as conversation, reading, light errands, or simply spending meaningful time together.

Bereavement support

Feelings of loss and grief can begin at the time of diagnosis and continue long after a loved one passes away. SOMC Hospice provides compassionate bereavement support throughout the hospice journey and for 13 months following the loss. Our trained bereavement counselor offers supportive phone calls and one-on-one grief counseling to help individuals and families navigate their grief.

Our bereavement program also provides grief counseling to community members, regardless of whether their loved one received hospice services.

24-hour on-call

SOMC Hospice provides 24-hour on-call support to meet the needs of our patients and families, day or night. An experienced registered nurse is available by phone to provide guidance and reassurance, and can make an in-person visit when needed to assess a patient and provide additional support.

Short-term inpatient care

SOMC Hospice offers an inpatient hospice unit designed to provide specialized care for patients who require additional support with pain and symptom management. The unit also provides respite care, offering patients and families an opportunity for additional support and comfort when needed.
